Risk Description
Manual operation of lifejacket requires force greater than standard allows to operate. Fault was identified by manufacturer as fault with FITTING of a particular “UML” Firing Head into this specific model. Tight packing of the outer cover can sometimes lead to a restriction of the manual operation pull cord. Where fitted, the automatic operation for inflation of these lifejackets is NOT affected. The inflation tag was subjected to the forces as required by EN 396:1993 part 6.8.4. and the firing head failed to operate at the maximum allowable force of 120 N (+2N).
