The adaptor poses a risk of electric shock and fire because: - its internal connections are not sufficient and rely upon solder to maintain their position; a loose wire could reduce creepage distances and clearances; - it has inadequately sized pins which could cause damage to the socket and the adaptor that could result in live parts being exposed.
