Risk Description
The heater had been constructed such that the elements are in contact with the live input terminal on the selector switch. This resulted in the elements being live even when the switch was in the off position and the fan not working. As soon as the heater was plugged in, the elements rapidly heated to red-hot. Surrounding plastics ignited and the heater caught fire.
