Risk Description
Child could fall as result of failure of the rings retaining the swing ropes to the frame. The rings attaching the swing ropes to the tubular steel frame were made from an inadequate grade of plastic, resulting in failure in use. Similar plastic were used in swing seats which also were found to fail, however as the rope passes under the seat child would not be allowed to fall in the event of swing seat failure. 11 incidents on loop, 8 on seat. Reported injuries include grazes, abrasions, cuts, sprained wrist.
