Risk Description
This product poses a serious risk of electric shock because the removal of the adaptor from the electric socket caused part of the adaptor to break off, exposing internal parts and posing a risk of electric shock. The risk can be posed by the UK, Australia and EU adaptor plugs. This product does not comply with the Low Voltage Directive.One incident was reported in the United Kingdom.

Product Description
The adaptor plugs are used with the AC Power Adaptor and supplied with 3108 Wireless Phone; are manufactured by DVE (Model DSA-10P-05). The Adaptor for use in the United States does not have the manufacturing defect and is safe to use, however, the UK, Australia and EU power plugs which were supplied with this wireless phone system have the reported defects. The same adaptor is shipped to all countries. There is one cord and power supply box, the various plug form factors snap on and off the power supply box. The US snap on adaptor is safe; the UK, Australian and EU adaptors had design and manufacturing defects. The affected power adaptor can be identified by the following markings: "DVE ® Switching Adaptor Model DSA-10P-05".
