The adapter poses a risk of electric shock and fire because:- the adaptor does not have adequate insulation between the input and output circuits; - the internal connections rely upon solder to maintain their position; a loose wire could reduce creepage distances and clearances;- it has inadequately sized pins which could cause damage to the socket and adaptor that could result in live parts being exposed.
