Risk of electric shock. Incompliant creepage distance and clearance between the mains circuit and the secondary circuit that can be touched; insufficient clearances / creepage between the primary and secondary windings, between the conductive patterns of the primary and secondary circuits; Insufficient resistance to fire of the plastic housing.
